Membership Plan Details & Terms
To keep our membership plan transparent and simple, please review the following details:
- Payment: Annual fees are due in full at the time of enrollment.
- Eligibility: For the Child Membership, patients must be 17 years or younger at enrollment.
- Exclusions: This plan cannot be combined with active dental insurance plans or additional promotional offers.
- Transferability: Memberships are non-refundable and non-transferable to other patients or dental offices.
